Direct Support Professional Position Description:
Duties:
Under the general supervision of the Direct Support Supervisor or Residential Director, supports people to achieve their highest level of independence.
Essential Functions
- Supports people based on information gained through conversations with the person and their plan of support. Develops a working relationship with people that provides individualized training in all skills based on the individual’s personal goals.
- Communicates effectively with other departments to ensure appropriate supports are addressed.
- Supports people to experience a variety of community options and settings.
- In coordination with the person and the plan manager, helps to develop goals and objectives based on the individual’s choice and assessments.
- Provides supports to people in the area of personal care and daily living skills.
- Assists persons being supported in learning appropriate behaviors through positive behavior support.
- Performs and supports people in the preparation of meals through individual choice and/or as outlined by agency or state regulations.
- Ensures the health safety, and accountability of people being served as indicated in agency policies. Conducts safety drills and inspections as required. Regularly discusses safety issues with people. Ensures that people have the best possible health.
- Supports people by attending life plan meetings when invited.
- Maintains transit safety through completion of transportation logs, daily vehicle checks, and adherence to agency safety policies.
- Maintains appropriate documentation as required.
- Provides a clean, safe, and respectful environment for people though support and training.
- Performs other related duties as required.
REQUIREMENTS:

HIGH SCHOOL DIPLOMA OR G.E.D.
-
ONE YEAR’S EXPERIENCE WITH SPECIAL NEEDS INDIVIDUALS AND ONE YEAR’S EXPERIENCE IN SUPERVISION OR AN ACCEPTABLE COMBINATION OF BOTH.

VALID SC DRIVER’S LICENSE WITH A GOOD DRIVING RECORD.

MUST BE ABLE TO LIFT 50 POUNDS

Community Training Home (CTH) / ICF Nurse
Full-Time
The Newberry County Disabilities and Special Needs Board (NCDSNB) is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Residential Services team. This position provides nursing care and oversight to individuals residing in our Community Training Homes (CTH) and Intermediate Care Facilities (ICF), ensuring quality healthcare services that promote health, safety, and independence.
Essential Duties

Provide nursing care to individuals residing in Community Training Homes (CTH) and Intermediate Care Facilities (ICF).

Ensure individuals receive appropriate medical treatment and ongoing health monitoring.

Administer medications and treatments in accordance with physician orders and agency policies.
-
Monitor and maintain accurate medical documentation.
-
Collaborate with physicians, pharmacies, residential staff, and interdisciplinary teams.
-
Assist the Lead LPN with nursing services and support other agency nursing needs as assigned.
-
Ensure compliance with DHEC, DDSN, and agency regulations and standards.
Minimum Qualifications
-
Current Licensed Practical Nurse (LPN) license in good standing with the South Carolina Board of Nursing.

Experience working with individuals with intellectual and developmental disabilities preferred.
-
Ability to work independently and effectively prioritize responsibilities.
-
Excellent communication and organizational skills.
-
Valid South Carolina driver's license with a good driving record.
